BackgroundPatients with lung cancer continue to smoke owing to complex factors. Failure to quit smoking (defined as nicotine dependence) is significantly associated with genetic status. This study aimed to investigate the relationship between polymorphisms in nicotine dependence genes and smoking status after the diagnosis of lung cancer.Patients and MethodsA total of 240 patients with lung cancer were included from July 2017 to March 2018. According to the actual smoking condition after lung cancer diagnosis, eligible patients were divided into 3 groups: the never-smoking group, the failure to quit smoking group, and the successful smoking cessation group. Fagerstrom Test for Nicotine Dependence scores were used to evaluate the smoking status of each group. Three nicotine-dependent genes with 6 loci were detected.ResultsAmong the 240 patients, 86 were never-smokers, 51 failed to quit smoking, and 104 successfully quit smoking. The initial age of smoking in the failure to quit smoking group was significantly younger than those in the successful smoking cessation group (P = .001). There was a significant difference in the GG and AG and AA genotype distributions of CHRNA3 (rs578776) among the 3 groups (P = .003). There was also a significant difference in the distribution of CHRNA4 (rs2229959) genotypes among the 3 groups (P = .003). However, there was no significant difference in the genotype distribution of CHRNA5 (rs588765) among the 3 groups (P = .277).ConclusionsGene polymorphisms of CHRNA3 (rs578776) and CHRNA4 (rs1044396 and rs2229959) were associated with the success of smoking cessation after the diagnosis of lung cancer, which should be considered in the management of smoking cessation after patients are diagnosed with lung cancer.  相似文献

目的:山白树是中国特有珍稀濒危植物。分析微卫星特征,开发其微卫星分子标记。方法:采用Illumina二代测序技术建立山白树基因组文库和微卫星文库,分析微卫星组成类型,设计山白树引物,用5个山白树种群进行扩增和检测以分析其多态性。结果:二代测序共返回38,942,660条100 bp配对序列,通过质量修剪及拼接得到200,386条拼接序列,其中长度 335 bp的为7 614条;在7 614条序列中检测出微卫星位点694个,其中单核苷酸重复最多,单核苷酸重复中A/T重复数量最多;二核苷酸长度变异最大,其中AG/CT重复数量最多,重复长度变异情况与微卫星丰度呈正相关。为36条山白树微卫星重复次数高的序列设计引物,经聚合酶链式反应(PCR)扩增和聚丙烯酰胺凝胶电泳检测,20对引物多态性丰富且条带清晰,等位基因数(NA)在3~6之间,平均为4,多态性信息含量(PIC) 0. 535 5~0. 754 0,平均值0. 615 5。对5个山白树种群的群体遗传分析发现,该物种遗传多样性较高(h=0. 697 5,I=1. 436 8,HE=0. 702 2),种群遗传分化显著(Fst=0. 374)。结论:通过部分山白树的群体遗传也可以说明本次开发的引物可用性较好。该研究开发了山白树微卫星分子标记引物,为山白树分子遗传学奠定了基础。  相似文献

Objective

Arch obstruction after the Norwood procedure is common and contributes to mortality. We determined the prevalence, associated factors, and practice variability of arch reintervention and assessed whether arch reintervention is associated with mortality.

Methods

From 2005 to 2017, 593 neonates in the Congenital Heart Surgeons' Society Critical Left Heart Obstruction cohort underwent a Norwood procedure. Median follow-up was 3.7 years. Multivariable parametric models, including a modulated renewal analysis, were performed.

Results

Of the 593 neonates, 146 (25%) underwent 218 reinterventions for arch obstruction after the Norwood procedure: catheter-based (n = 168) or surgical (n = 50) at a median age of 4.3 months (quartile 1-quartile 3, 2.6-5.7). Interdigitation of the distal aortic anastomosis was protective against arch reintervention. Development of ≥ moderate tricuspid valve regurgitation and right ventricular dysfunction at any point was associated with arch reintervention. Nonsignificant variables for arch reintervention included shunt type and preoperative aortic measurements. Surgical arch reintervention was protective against arch reintervention, but transcatheter reintervention was associated with increased reintervention. Arch reintervention was not associated with increased mortality. There was wide institutional variation in incidence of arch reintervention (range, 0-40 reinterventions per 100 years patient follow-up) and in preintervention gradient (range, 0-64 mm Hg).

Conclusions

Interdigitation of the distal aortic anastomosis during the Norwood procedure decreased the risk of arch reintervention. Surgical arch reintervention is more definitive than transcatheter. Arch reintervention after the Norwood procedure is not associated with increased mortality. Serial surveillance for arch obstruction, integrated with changes in right ventricular function and tricuspid valve regurgitation, is recommended after the Norwood procedure to improve outcomes.  相似文献

目的:分析CYP2C19*3基因多态性与癫痫患者奥卡西平(oxcarbazepine,OXC)活性代谢产物10,11-二氢-10-羟基卡马西平(monohydroxycarbazepine,MHD)血药浓度的相关性。方法:纳入120例OXC单药治疗1个月以上且症状控制良好的癫痫患者,采集清晨服药前空腹血,采用高效液相色谱法测定MHD稳态谷浓度。通过PCR和sanger测序判定患者CYP2C19*3基因型。结果:120例癫痫患者快代谢患者64例,MHD血浆浓度为(18.17±7.34)μg·mL-1;中代谢型患者36例,MHD血浆浓度为(19.31±9.17)μg·mL-1;慢代谢型患者20例,MHD血浆浓度为(25.79±7.51)μg·mL-1,3种基因型的MHD浓度有显著性差异(F=7.077,P=0.0013)。多因素分析显示,日剂量作为影响血药浓度的重要指标呈现出显著相关性(P<0.05)。结论:CYP2C19*3基因多态性影响MHD血药浓度,并且日剂量越高,MHD血药浓度越大,两者成显著正相关,临床应进行血药浓度监测。  相似文献

Job stress and the Circadian Locomotor Output Cycles Kaput (CLOCK) gene could affect circadian rhythm and sleep quality. The main aim of our present study was to investigate the association of job stress, CLOCK gene polymorphism and their interaction with sleep quality in a non‐clinical Chinese Han population, which has not been reported to date. Using a cross‐sectional design, 450 subjects were recruited in Beijing. Sleep quality was measured with the Pittsburgh Sleep Quality Index (PSQI) and job stress was measured with the Work Stress Scale. CLOCK gene rs11932595 polymorphism was genotyped in 297 blood samples. Correlation analysis showed a close but different association of high job stress with the PSQI and its components. Analysis of variance showed significant main effects of the CLOCK gene rs11932595 polymorphism. G‐allele carriers had a higher score in the PSQI, sleep duration, sleep latency and sleep disturbances. Further interaction analyses showed an ordinal interaction on sleep duration, and a disordinal interaction on daytime dysfunction. Specifically, G‐allele carriers had poorer sleep duration than AA homozygotes when in high job stress, while the two subgroups displayed similar sleep duration when in low job stress, conforming to the diathesis–stress model. In comparison to G‐allele carriers, AA homozygotes experienced less daytime dysfunction when in low job stress whereas more daytime dysfunction when in high job stress, fitting with the differential susceptibility model. As genetic links have been revealed, our investigation might be conducive for elucidating aetiological factors for sleep quality and targets for implementing interventions to attain good sleep quality.  相似文献
